The vehicle conveying the late monarch and about a dozen other traditional rulers somersaulted and skidded off the road into the bush near Sagbama community. While most of the victims received first aid, those with severe injuries were stabilised and referred to the Niger Delta Teaching Hospital at Okolobiri. Tragically, HRH Onakpohor succumbed to his injuries, leaving the Ofoni community and Bayelsa State bereft of a revered leader.

Upon receiving the tragic news, Governor Diri acted swiftly, canceling the planned event in Sampou out of respect for the fallen monarch and the victims of the accident.
